How to Get a Tensas Parish Birth Certificate
Because the Tensas Parish Clerk is not an authorized issuance point, the primary source for certified birth certificates is the Louisiana Vital Records Registry. This state office handles birth records for everyone born in Louisiana since statewide registration began in 1911. Walk-in service is available at 1450 Poydras Street, Suite 400, New Orleans, LA 70112. The main phone is (504) 593-5100. Walk-in hours run from 8:00 AM to 3:30 PM on weekdays, excluding state holidays. The call center is available at (504) 219-4500 from 8:00 AM to 4:30 PM.
If you cannot travel to New Orleans, mail-in orders are accepted. You can download the application form from the LDH website at ldh.la.gov/page/how-to-order-birth-records. Mail orders typically take 8 to 10 weeks to process. Send your completed application, a copy of your photo ID, and payment to the mailing address: P.O. Box 60630, New Orleans, LA 70160.
Online orders are also available. VitalChek at vitalchek.com is the authorized third-party service. You can also call VitalChek at 1-877-605-8562. Processing fees apply in addition to the certificate cost when ordering online or by phone. Online orders are usually faster than mail-in requests to the state office.
Note: Complimentary certified copies are provided to parents of newborns by the state shortly after the birth record is registered. If you have not received it within four months of your child's birth, call (504) 593-5100.
Eligibility for Tensas Parish Birth Records
Louisiana keeps birth records closed for 100 years under LSA R.S. 40:36. They are not public records. Only people who fall within the state's eligibility rules can request a certified copy. This applies whether you are requesting from the state office or through VitalChek.
Eligible requestors include the person named on the birth certificate, their current legal spouse, the mother or father of the person named, an adult child, a sibling, a grandparent, a grandchild, a licensed attorney representing one of the above, and a legal guardian with a court judgment of custody. A notarized statement of guardianship does not satisfy the requirement. You need the actual court order. Step-parents, cousins, in-laws, and ex-spouses are not eligible under Louisiana law.
You must provide valid identification with your request. One primary document is sufficient. Acceptable primary ID includes a current state driver's license or picture ID, a U.S. military identification card, a U.S. or foreign passport, or a Certificate of Naturalization or Citizenship. If you use secondary documents, you need two. Secondary options include a W-2 form with a Social Security card, a payroll stub with a Social Security number, a voter registration card, property deeds, or an insurance policy.
Under LSA R.S. 40:61, making false statements on a vital records application is a criminal offense. It can result in a fine of up to $10,000, imprisonment of up to five years, or both. This applies to any application, whether filed in person or submitted by mail or online.
State Birth Certificate Fees
At the Louisiana Vital Records Registry, a long-form birth certificate costs $15.00. A birth card (short form) is $9.00. If you order both at the same time, the pair is $24.00. These are the state-level fees. If you order through VitalChek, additional processing fees apply on top of these amounts.
The LDH directory page at ldh.la.gov/directory/category/279 lists all authorized clerk offices across Louisiana. Since Tensas Parish is not on that list, the state office is the correct channel for birth certificate requests from this parish.
The LDH directory helps identify which parishes have local clerk access for birth records and which ones, like Tensas, need to use the state office directly.
Note: Fee amounts are set by the Louisiana legislature and can change. Always confirm the current fee with the state office before submitting your request or including payment.
